Why concentrated 7-OH products are different from natural kratom leaf
Kratom leaf naturally contains a little bit of 7-OH, but not much—less than 2%, according to the FDA. Commercial 7-OH products are made to contain far more of it than that.
Manufacturers get there one of two ways: pulling 7-OH straight out of the plant and concentrating it, or chemically converting other kratom compounds into 7-OH in a lab. Either method leaves you with something far stronger than plain kratom leaf, chewed or brewed. This is a big part of why concentrated 7-OH products act more like a strong opioid than a mild plant-based product.

Short-term effects of 7-OH
7-OH can affect people differently depending on how much you take, how often you use it, and the strength of the product. Some effects may feel pleasant at first, while others can be uncomfortable or potentially dangerous.
Short-term effects include:
- Pain relief, relaxation, or euphoria
- Drowsiness, nausea, constipation, and itching: common with opioids in general, not unique to 7-OH.
- Slowed or shallow breathing, especially at higher doses: the most serious risk. It becomes a lot more likely when 7-OH is mixed with alcohol, benzodiazepines, or other sedatives.
- Rebound anxiety, irritability, or a “crash” feeling as the effects wear off: a pattern that can make it tempting to use again sooner than you planned.

Respiratory and liver risks

The first is respiratory depression, a serious medical condition where breathing becomes abnormally slow and shallow, which is the most dangerous thing that can happen with any opioid. It becomes far more likely when 7-OH is mixed with alcohol, benzodiazepines, or other sedatives.
The second is liver damage. Chronic recreational use of kratom or 7-OH has been associated with rare instances of acute liver injury, typically appearing within 1 to 8 weeks of starting regular use, with symptoms including fatigue, nausea, itching, and dark urine progressing to jaundice. In one reported case, a person developed a serious liver injury after using kratom, confirmed through medical testing, with everything pointing to kratom as the cause.
Both of these risks have been documented in real cases. And while the evidence is limited, it doesn’t make the risk any less real. If you’re experiencing trouble breathing, extreme sleepiness, or other severe symptoms after taking 7-OH or kratom, seek emergency medical care immediately.
Addiction, tolerance, and why 7-OH can be hard to stop
